You are responsible for knowing what items are required for your team. Soccer City is not responsible for orders submitted incorrectly. Please also review our Return Policy for custom printed items before submitting orders.
Click on the appropriate link for your club. The file will open up in a new window. DOWNLOAD the form to your computer. Then open it in Adobe Reader. You should now be able to fill it out on your computer. Remember to SAVE A COPY of your completed form to your computer, or you may lose your work! Attach the form to an email and send it to SoccerSavesInfo@frontier.com. Be sure to include any special instructions or timeframes in the email.
